I have had this issue before, but I now have to resolve it. I have a zenoss 4 install that does not seem to want to load MIBs.
I do "zenmib run -v10
"Found 1 MIBs to import" & "Loaded 0 MIB file(s)" and no error messages whatsoever.
How do I debug further

There was an append I wrote way back but since Zenoss dropped the old forum it has now gone :(
I believe it was a timing thing. Eventually "it just worked".
I haven't seen this issue now for a long time and am now on 4.2.5 SUP 671 - how about you

The "randomness" is not to do with the installation but with running zenmib.py - there is a race condition in that code somewhere.
So I have finally found my old solution - it's actually documented in the Event Management paper here https://www.skills-1st.co.uk/papers/jane/zenoss4-events/zenoss_Core4_event_management_paper.pdf. I went back to an old (at that time, Zenoss 3.1 version) of zenmib.py.
I find that my current 4.2.5 SUP 671 system also has a copy of that old file and I have just tried replacing the current zenmib.py with the old one (obviously having backed up the current one). For me, on one test, the old one still works.
If you have a test system, you might try it - at least while you do the import of your problem MIB.
I know I spent ages trying to find out what was going wrong and never really got anywhere :(

Be interested to know if it works for you if you have an environment where you are able to try it.
